DSI offers case management services to children and adults with developmental disabilities and adults with acquired brain injury. Individual Services Coordinators take a personal approach to assisting persons served with Waiver Eligibility and the Planning Process.
Five (5) Individual Services Coordinators (also referred to an ISC's or Case Managers) assist participants and their families in coordinating services, accessing various resources, monitoring service delivery, assisting with conflict resolution, crisis intervention and resolving other issues as needed. ISC's work with other service providers in the surrounding area to develop a Plan of Care that best meets the needs of the person served.
A major component of the planning process is the DREAMS section, which is discussed on an annual basis at the Six Month Plan of Care Review for the participants. The participant communicates dreams he/she may have, and a responsible support person and target completion date is set to help support the person in achieving his/her dreams. Some examples of dreams that have been established and accomplished include attending major sporting events such as Colorado Rockies baseball games, going for airplane rides, learning to drive, going on fishing/hunting/camping trips, going horseback riding, and trips to the Black Hills, Hawaii, and Florida.
